Язвенная болезнь является традиционной проблемой, которая находится в центре внимания клиники внутренних болезней (26). Последние 20-30 лет исследования этой патологии ознаменовались значительным прогрессом в области изучения этиологии, патогенеза и лечения. В терапии язвенной болезни активно применяются Нг - блокаторы, ингибиторы протонной помпы, антибактериальные препараты. Казалось бы, опасный недуг должен быть побежден или, по крайней мере, находиться под полным контролем врачей. Но этого не происходит. Частота осложнений язвенной болезни в России за последние 30 лет фактически не претерпела изменений. В 1973 г. Г.И. Бурчин-ский, В.Е. Кушнир при-изучении 1000 историй болезни обнаружили информацию об язвенных кровотечениях у 19,6%, а о перфорациях - у 7% пациентов (11). В 1987 г. В.Х. Василенко и соавт. сообщают о частоте кровотечений 10 - 15%, перфораций - 8 - 10% (13). В 2001 г. П.Е. Соколович и соавт. докладывают о том, что кровотечения встречаются у 10 — 20% больных с язвами, а перфорации - у 10 - 15% пациентов (60). С нашей точки зрения, причиной этого является неадекватная диспансеризация. По данным О.Н. Минушкина и соавт. (45), в России находятся на диспансерном наблюдении около I млн. человек, что в пересчете на население страны дает распространенность около 1%. Между тем реальная распространенность язвенной болезни по результатам исследований в США и Европе колеблется от 6,5% до 10% (443). Это означает, что более 80%) пациентов с язвенной болезнью в России остается без необходимого наблюдения и квалифицированной помощи.

Изменить эту неблагоприятную ситуацию могут только эпидемиологические исследования. Вместе с тем в нашей стране подобные работы весьма немногочисленны. По сути дела, только ученые из Красноярска (74) и Новосибирска (37) проявляют активность в этой области. В этой связи данное исследование представляется чрезвычайно актуальным.

В последние годы большое внимание привлекает проблема диспепсии. Постоянно совершенствуется ее классификация, предпринимаются эпидемиологические исследования, изучается связь с язвенной болезнью, инфекцией Helicobacter Pylori (181). Одной из наиболее значительных и важных для практических врачей является стратегия «Test and treat», принятая Американской гастроэнтерологической ассоциацией в 2000 г. и поддержанная Маастрихтом-2 в 2000 г. Эта стратегия заключается в диагностике HP неин-вазивным методом с последующим назначением эрадикации без эндоскопии и используется у взрослых пациентов с упорной диспепсией моложе 45 лет. Подход «Test and treat» может быть полезен в организации мероприятий по профилактике язвенной болезни и рака желудка (96). Но для рекомендаций по использованию этой стратегии необходимо доказать наличие ассоциации диспепсии с гастродуоденальной патологией и инфекцией HP. Актуальность этой проблемы чрезвычайно высока, но подобных исследований в России практически нет.

Прогресс в изучении язвенной болезни в настоящее время в значительной степени связан с исследованием инфекции HP. Вместе с тем до сих пор не завершена дискуссия о роли бактерий* в ульцерогенезе. Например, в ряде европейских работ показана ассоциация VacA и CagA штаммов HP с язвенной болезнью (312), но в исследованиях из Азии подобная закономерность нередко отрицается (367). В России известны только два сообщения из Ставрополя, и Москвы (343), авторы которых не нашли взаимосвязи цитотоксиче-ских штаммов HP с патологией.

В последние годы активно развивается направление этно-экологической гастроэнтерологии. Выполнен ряд крупных исследований в различных регионах Восточной Сибири, которые позволили обнаружить различия клинических проявлений гастроэнтерологической патологии у популяций монголоидов и европеоидов (72). Важность этого направления состоит в том, что клинические дисциплины развиваются, как правило, вне основных положений экологии. Популяционный подход нетипичен для современной физиологии и мало применяется для изучения заболеваний (64). Между тем Россия характеризуется огромными размерами, резко отличающимися условиями жизни в различных регионах и разнообразием этнического состава населения. Выполненное исследование посвящено изучению одного из наиболее значимых аспектов патологии пищеварительного тракта в рамках системного развития направления этно-экологической гастроэнтерологии.

Цель исследования:

Изучить взаимосвязь клинико-эпидемиологических проявлений язвенной болезни с диспепсией, инфекцией Helicobacter Pylori, морфологической структурой слизистой оболочки желудка у европеоидов и монголоидов Республики Хакасия для совершенствования лечебно-профилактических мероприятий.

Задачи исследования:

1. Исследовать распространенность, клинические проявления'и факторы риска язвенной болезни у европеоидов и монголоидов.

2. Изучить распространенность, структуру и взаимосвязь диспепсии с га-стродуоденальной патологией у европеоидов и монголоидов.

3. Установить показатели обсемененности, распределение цитотоксиче-ских штаммов HP и ассоциацию бактерий с диспепсией и язвенной болезнью у европеоидов и монголоидов.

4. Изучить морфометрическую структуру и формулу желез слизистой оболочки желудка у пациентов с язвенной болезнью среди европеоидов и монголоидов.

5. Исследовать показатели клеточного обновления эпителиоцитов и им-мунофенотип клеточного инфильтрата собственной пластинки в слизистой оболочке желудка у пациентов с язвенной болезнью среди европеоидов и монголоидов.

Научная новизна исследования:

Выполнено масштабное исследование распространенности язвенной болезни у различных популяций населения с позиции экологической гастроэнтерологии на юге Восточной Сибири.

Установлены отличия распространенности и клинических проявлений язвенной болезни и диспепсии у европеоидов и монголоидов Республики Хакасия.

Распределение цитотоксических штаммов НР и ассоциация бактерий с диспепсией и язвенной болезнью имеет выраженные особенности у европеоидов и хакасов.

Впервые изучение распространенности патологии у двух различных популяций сочеталось с морфометрическим, гистохимическим и иммуногисто-химическим изучением слизистой оболочки желудка у лиц с язвенной болезнью.

Атрофические изменения в слизистой оболочке антрального отдела желудка и относительная гиперплазия обкладочных клеток в теле желудка у пациентов с язвенной болезнью имеет отличия у европеоидов и хакасов.

Для язвенной болезни двенадцатиперстной кишки характерно превалирование активности апоптоза над пролиферацией эпителиоцитов прежде всего в антральном отделе желудка, которое имеет отличия среди обследованных популяций.

Впервые проведено сравнительное изучение иммунофенотипа клеточного инфильтрата собственной пластинки у различных популяций населения, показавшее очевидную дифференциацию иммунного ответа у лиц с язвенной болезнью среди европеоидов и хакасов.

Практическая значимость работы:

1. Высокая распространенность диспепсии, язвенной болезни и НР обосновывает целесообразность дальнейшего развития гастроэнтерологической службы у населения Сибири.

2. Результаты работы позволяют планировать организацию медицинской помощи населению на основании фактической информации.

3. Повысит эффективность диспансеризации населения в связи с установлением факторов риска и особенностей течения патологии.

4. Результаты изучения лекарственной резистентности НР к антибиотикам у жителей Республики Хакасия обосновывает выбор лекарственных препаратов для рациональной терапии язвенной болезни.

5. Доказательство ассоциации показателей обсемененности НР и кисло-топродукции в желудке у населения Республики Хакасия верифицирует идею о важности эрадикации бактерий для лечения язвенной болезни.

Внедрение в практику:

Материалы исследований утверждены в качестве методических пособий и внедрены в практику в Министерстве здравоохранения Республики Хакасия, Хакасской республиканской больнице, в Таштыпской ЦРБ (Хакасия), Хакасском государственном университете, городской больнице г. Абакана, НИИ медицинских проблем Севера СО РАМН.

Апробация работы:

Материалы диссертации докладывались на 6-й, 7-й, 8-й, 9-й Российских гастроэнтерологических неделях- (Москва, 2000 - 2003 гг.), на 24-й, 25-й, 27-й сессиях ЦНИИГ (Москва, 1997 г., 1998 г., 2000 г.), 5-м Славяно-Балтийском гастроэнтерологическом форуме (С.-Петербург, 2003 г.), 11-м международном конгрессе по приполярной медицине-(Харстад, Норвегия, 2000 г.), 15-й международной конференции по-изучению гастродуоденальных заболеваний и Helicobacter (Афины, Греция, 2002 г.), 11-й Европейской гастроэнтерологической неделе (Мадрид, Испания, 2003 г.), 5-й, 6-й, 10-й Сибирских гастроэнтерологических конференциях (Томск, 1997 г., 1998 г., 2002 г.), 1-й, 2-й, 3-й, 4-й Восточно-Сибирских гастроэнтерологических конференциях (Красноярск, 2000 г., 2002 г.; Абакан, 2001 г., 2003 г.).

По материалам исследований опубликованы 32 печатные работы, в том числе две монографии.

Положения, выносимые на защиту:

1. Распространенность язвенной болезни и диспепсии, клиническое течение язвенной болезни имеют значительные отличия у европеоидов и монголоидов Республики Хакасия.

2. С язвенной болезнью и диспепсией у европеоидов .и хакасов ассоциированы различные типы,и субтипы HP.

3'. В обследованных группах населения язвенная болезнь двенадцатиперстной кишки сопровождается однонаправленными изменениями структуры слизистой оболочки желудка, проявляющиеся в развитии атрофии в аптраль-ном отделе и увеличении плотности желез, доли железистого эпителия и доли обкладочных клеток в теле желудка, но эти процессы выражены в большей степени среди европеоидов в сравнении с хакасами.

4. Активность пролиферации и апоптоза слизистой оболочки желудка, обеспечивающие уровень клеточного обновления эпителиоцитов, иммунофе-нотип клеточного инфильтрата в собственной пластинке ассоциированы с язвенной болезнью и имеют отличия в популяциях европеоидов и хакасов.

Объем и структура диссертации:

Диссертация изложена на 287 страницах машинописного текста и состоит из введения, обзора литературы, описания материалов и методов, собственных исследований, заключения,-выводов-и практических рекомендации.

4. Результаты исследования резистентности HP к антибиотикам обосновывают недопустимость применения в схемах эрадикации бактерий метро-нидазола в Республике Хакасия. Отсутствие резистентности HP к кларитро-мицину позволяет считать возможным применять суточную дозу данного препарата равную 500 мг. Оптимальной схемой стандартной семидневной тройной терапии в обследованном районе является сочетание ингибитора протонной помпы с кларитромицином и амоксициллином.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Низкая осведомленность больных людей о наличии язвенной болезни, высокая распространенность заболевания указывает на целесообразность применения активных эпидемиологических методов выявления патологии и интенсивного развития гастроэнтерологической службы у населения Сибири. Подход к диагностике и лечению язвенной болезни у европеоидов и монголоидов Сибири должен быть дифференцированным.

2. Основными факторами риска язвенной болезни у населения Республики Хакасия являются инфекция HP и курение табака. Борьба с факторами риска должна быть ведущей стратегией профилактики ульцерогенеза в данном регионе.

3. Ассоциация диспепсии с гастродуоденальной патологией и инфекцией Helicobacter Pylori свидетельствует о целесообразности внедрения стратегии «Test and treat» у населения Республики Хакасия для профилактики язвенной болезни.
