Повреждения связочно-капсульного аппарата занимают одно из первых 'ест среди всех повреждений суставов и приводят к тяжелым поражений соединительно-тканных элементов коленного сустава ([55], [65], [69], [100], [105], [309], [245]). По данным [8], [21], [68], [128], [129], [151], [207], [267], [415], [439], [635] и многих других, в травматологической практике преобладают множественные разрывы элементов связочно-капсульного аппарата. По мнению ([641], [647], [653]), практически всегда происходит повреждение нескольких связок коленного сустава, при этом основной контингент пострадавших - люди трудоспособного возраста [4], [47], [69], а сроки временной их нетрудоспособности достигают 8-9 месяцев [385], [433]. Кроме того, повреждения связочно-капсульного аппарата коленного сустава и их последствия характеризуются высокой инвалидизацией посградавших ([154], [605]), обусловленной запоздалой диагностикой этих повреждений. Почти 45% пациентов поступает в стационар с застарелыми повреждениями и хронической нестабильностью коленного сустава ([21], [68], [72]). Так, по данным [376], в остром периоде травмы не диагностируются 80% повреждений малоберцовой коллатеральной связки. По данным [47], [74], правильный топический диагноз повреждения связок коленного сустава ам-булаторно был установлен только у 20-40% пострадавших. В работах [6], [96], [137], [147], [737] показано, что основная часть больных с повреждением связок коленного сустава поступает на лечение в поздние сроки после травмы - от 6 месяцев до 6 лет.

Существующие методы хирургическо1 о лечения этих повреждений можно подразделить на восстановительные операции (шов, рефиксация - [21], [74], [75], [ 152]), реконструктивные (ауто- и аллопластическое замещение, эндопротезиро-вание - [55], [68], [69], [90], [88], [100], [140]) и консервативное лечение поражений [334], [336].

Тем не менее, единодушия в выборе тактики лечения этих повреждений все еще нет [113], [131], [144], [146]. Отсутствие результатов экспериментального исследования морфо-функциональной организации сухожильных и связочных элементов на различных структурных и ультраструктурных уровнях их строения в норме и патологии не позволяет построить целостную систему диагностики, лечения и определения прогноза исходов поражений связочных элементов свя-зочно-капсулъного аппарата коленного сустава [26].

В настоящее время предметом дискуссии остается выбор рекомендаций в отношении показаний к операциям [113], [590], так, например, до настоящего времени находят применение такие консервативные методы лечения острых повреждений, как длительная внешняя иммобилизация гипсовой повязкой ([323], [744]). Не существует единого экспериментального и биомеханического обоснования методов хирургического лечения повреждений связочно-капсульного аппарата коленного сустава [77], [87], [113], [334], [509]. Остаются дискутабель-ными вопросы выполнения первичного шва и рефиксации оторванных связок [74], [79], [87], рекомендаций к первичному укреплению шва и первичной реконструкции связок за счет аутопластического замещения связки или ее эндопроте-зирования [21], [115], показаний к артроскопическому или артротомическому типу вмешательства [75], [99], [113].

Существующие способы шва, ауто-, аллопластики и эндопротезирования при повреждениях боковых связочных комплексов часто не в состоянии обеспечить стабильность коленного сустава, особенно при сочетании этих разрывов с повреждениями крестообразных связок ([21], [113], [119], [327] и др.).

Остаются малоизученными особенности лечения при множественных разрывах связок и вывихах голени, разрывах связок, сочетанных с внутрисуставными переломами, переломами диафизов бедра и голени на одной стороне и других поражений [21], [105], [137].

Рекомендации по организации программы реабилитации после восстановительных операций по поводу острых повреждений связочных элементов коленного сустава противоречивы - от длительной гипсовой иммобилизации с запрещением нагрузки на ногу [110], [138] - до начала активных и пассивных движений в суставе на 2- 3 день после операции с отказом от наложения гипсовой повязки [85], [148], [674].

Одной из фундаментальных причин обсуждаемых проблем является отсутствие знаний о морфо-функциональной организации [59] соединительно-тканных элементов и общих закономерностях репаративных процессов в соединительных тканях при различных типах повреждений и их оперативном лечении.

Вышеизложенное обосновывает актуальность избранной темы исследования и определяет необходимость клинико-экспериментального обоснования лечения острых повреждений крестообразных и коллатеральных связок коленного сустава.

Цель работы.

Создать эффективную систему хирургической коррекции повреждений крестообразных и коллатеральных связок коленного сустава в остром периоде травмы, направленной на полное и раннее восстановление функции поврежденного сустава.

Задачи исследования:

1. Определить информативность применяемых диагностических мероприятий при острых повреждениях крестообразных и коллатеральных связок коленного сустава и разработать алгоритм их использования.

2. Определить прикладное значение морфо-функциональных закономерностей и особенностей физиологии связок коленного сустава для объективизации хирургической тактики при острых повреждениях связочно-капсульного аппарата коленного сустава.

3. Разработать новые объективные методы оценки состояния элементов связочно-капсульного аппарата коленного сустава на основе применения современных видеокомпьютерных технологий количественной оценки морфо-функциональных закономерностей существования биомеханических систем в норме, при различных типах нарушения функции и экспериментальных оперативных вмешательствах.

4. Разработать показания к хирургическому и функциональному лечению острых повреждений крестообразных и коллатеральных связок коленного сустава различного характера и локализации.

5. Усовершенствовать хирургические способы первичного восстановления и пластического замещения крестообразных и коллатеральных связок коленного сустава в остром периоде травмы.

6. Разработать оптимальные комплексы функционального лечения повреждений крестообразных и коллатеральных связок коленного сустава и лечебные мероприятия в пред-, послеоперационном и реабилитационном периодах.

7. Исследовать отдаленные результаты лечения острых повреждений крестообразных и коллатеральных связок коленного сустава и провести анализ осложнений и ошибок.

Научная новизна исследования.

На основе изучения морфо-функциональных закономерностей репаратив-ных процессов в сухожильной ткани и ткани связок разработан и внедрен в кли-нико-экспериментальную практику способ объективизации оценки эффективности восстановительных и реконструктивных оперативных вмешательств при повреждениях соединительно-тканных элементов связочно-капсульного аппарата коленного сустава.

В результате проведенного исследования:

1. Впервые построена математико-биомеханическая модель изотропной трансверзальной среды связки надколенника и крестообразной связки с формированием структуры тензора напряжений и тензора деформаций, позволяющая объективизировать выбор тактики лечения повреждений крестообразных связок коленного сустава в остром периоде травмы.

2. Впервые разработаны комплексные алгоритмы выбора тактики лечения крестообразных и коллатеральных связок коленного сустава в остром периоде травмы.

3. Разработан ьовый способ артроскопической рефиксации крестообразных связок коленного сустава в остром периоде травмы.

4. Доказано, что метод аутопластики передней крестообразной связки коленного сустава трансплантатом из связки надколенника с костными блоками на концах в 24,4±6,4% приводит к формированию резидуальной нестабильности коленного сустава из-за остеопластических изменений внутрикостных каналов.

5. Выявлены новые морфо-функциональные закономерности репаративной регенерации соединительно-тканных элементов (сухожилий, связок) связочнокапсулыюго аппарата коленного сустава и впервые разработан комплекс информативных морфоденситометрических показателей состояния элементов соединительной ткани в норме и патологии.

Практическая значимость и реализация результатов.

Значимость работы определяется комплексным подходом к решению проблемы улучшения результатов лечения повреждений крестообразных и коллатеральных связок коленного сустава в остром периоде травмы. Разработаны показания к хирургической коррекции и консервативному функциональному лечению острых повреждений, определены характер и особенности выполнения восстановительных и реконструктивных операций в зависимости от вида, локализации и степени поражения ткани связок .

Построены алгоритмы выбора тактики лечения повреждений крестообразных и коллатеральных связок коленного сустава в зависимости от степени поражения, выраженности диагностических признаков, изолированного или множественного характера повреждений.

На основе анализа результатов применения клинического и рентгенологического методов диагностики, методов компьютерной магнитно-резонансной томографии и артроскопии сформирована система диагностических мероприятий по исследованию острых повреждений связок коленного сустава, позволяющая повысить доказательность диагностических решений.

В экспериментальной модели при сравнении результатов различных типов восстановительных и реконструктивных операций на передней крестообразной связке с комплексным использованием световой и сканирующей электронной микроскопий, видео-информационной технологии цифровой обработки изображений - компьютерной морфоденситометрии произведено ранжирование эффективности различных оперативных вмешательств. Клинически и экспериментально доказана возможность сохранения жизнеспособности крестообразных связок при их острых отрывах от мест прикреплений в случае их рефиксации.

С учетом анализа выявленных морфо-функциональных закономерностей репаративных процессов в связочной и сухожильной ткани разработаны интегральные количественные морфо-денситометрические показатели архитектоники элементов соединительной ткани, позволяющие объективизировать выбор тактики лечения и прогноз его исхода.

Апробация работы и внедрение в практику.

Предложенная тактика лечения острых повреждений связочно-капсульного аппарата коленного сустава в остром периоде травмы успешно применяется в городских клинических больницах №1, №3, №33, №64, детской городской клинической больнице Св. Владимира Департамента здравоохранения г. Москвы.

Материалы диссертации доложены и обсуждены в 12 докладах на 2 конгрессах, 2.съездах. 5 конференциях, и т.п., опубликованы в 45 научных статьях, 1 пособии для врачей.

Зарегистрированы 2 рационализаторских предложения отраслевого значения №0-3694 и № 0-3695.

Объем и стру ктура диссертации

Диссертация состоит из введения, 6 глав, заключения, выводов, практических рекомендаций, указателя литературы, включающего 775 источников, в том числе 158 на русском и 617 на иностранных языках.

ВЫВОДЫ:

1. Установленные информативность и диагностическая ценность (чувствительность, специфичность, точность) методов клинической диагностики (92,85%; 50,1%; 78,1% соответственно), магнитно-резонансной томографии (83%; 95%; 93,3%) и артроскопии (97,3%; 86,6%; 95,02%) при острой травме связочно-капсульного аппарата коленного сустава обосновывают формирование системы диагностических мероприятий по исследованию острых повреждений крестообразных и коллатеральных связок коленного сустава и построение алгоритма применения рентгенологического, клинического методов, магнитно-резонансной томографии и артроскопии.

2. Разработанные новые методы оценки морфологических и функциональных изменений в связочно-капсульном аппарате коленного сустава, основанные на комплексном использовании световой и сканирующей электронной микроскопий, видео-информационной технологии цифровой обработки изображений -компьютерной морфоденситометрии и сформированного интегрального количественного показателя упорядоченности субфибриллярных и фибриллярных соединительно-тканных элементов связочно-капсульного аппарата коленного сустава, позволяют объективизировать оценку результатов реконструктивных и восстановительных операций при повреждениях крестообразных связок.

3. Обнаруженные и количественно характеризованные на основе компьютерной морфоденситометрии закономерности восстановления морфо-функциональной целостности связочно-капсульного аппарата коленного сустава - топология и динамика числа субфибриллярных элементов гистоархитектоники (коннекторных элементов) позволяют прогнозировать исход реконструктивно-восстановительных методов хирургического лечения в зависимости от его характера (восстановительное, реконструктивное).

4. Разработанные абсолютные и относительные показания для хирургического лечения травм коллатеральных связок и стабилизаторов наружного отдела коленного сустава определяются характером и локализацией повреждений, возрастом и степенью физической активности пациентов; показания к функциональному консервативному лечению - наличием частичных и изолированных разрывов крестообразных связок и стабилизаторов внутреннего отдела коленного сустава.

5. Результаты различных хирургических вмешательств по восстановлению передней крестообразной связки могут быть систематизированы и ранжированы (р<0,01) на основе интегрального показателя энтропии субархитектоники: I. ре-фиксация связки после отрыва от места прикрепления к болыпеберцовой кости > II. рефиксация связки после отрыва от места прикрепления к бедру > III. аутопластика связки несвободным лоскутом из связки надколенника > IV. аутопластика связки свободным лоскутом из связки надколенника. Возможность сохранения жизнеспособности крестообразных связок при их острых отрывах от мест прикреплений в случае их рефиксации доказана клинически и экспериментально (р<0,05).

6. Усовершенствованные хирургические способы первичного восстановления - артроскопической рефиксации крестообразных связок, в т.ч. с фрагментом межмьпцелкового возвышения болыпеберцовой кости, шва внутренних отделов капсулы и стабилизирующих элементов внутреннего отдела сустава под артрос-копическим контролем) и реконструкции крестообразных и коллатерачьных связок коленного сустава характеризуются надежностью, технической простотой и высокой эффективностью.

7. Комбинация изометрических напряжений мышц бедра и голени, активных и пассивных движений в суставе, дозированной нагрузки на конечность и формирования идеомоторных проприоцептивных связей, составляющая основу разработанной системы функционального консервативного лечения острых повреждений крестообразных и коллатеральных связок коленного сустава, реабилитационных мероприятий в пред- и послеоперационном периодах определяется характером и локализацией повреждений.

8. На основе изучения отдаленных результатов лечения повреждений крестообразных и коллатеральных связок коленного сустава установлено, что применение усовершенствованных методов диагностики, хирургического и функционального лечения позволяет получить хорошие исходы по шкале ЬузЬо1т у наиболее тяжелой группы пострадавших с множественными повреждениями связок у 66,7% больных, удовлетворительные исходы - в 23,8% и неудовлетворительные - в 9,5% наблюдений.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

1. Выполнение артроскопии у больных со свежим посттравматическим гемартрозом позволяет уменьшить число ложно-отрицательных результатов диагностики повреждений внутрисуставных элементов сустава, что позволяет рекомендовать применение этого метод в качестве элемента комплексной диагностики СКА КС.

2. МРТ следует применять в качестве основного информативного метода диагностики при исследовании больных с подозрением на перенесенный вывих голени; в связи с возможностью развития интраоперационных осложнений (т.н. "футлярного синдрома") применение артроскопии должно быть ограничено.

3. Выбор показаний к оперативному или консервативному лечению определяется результатами многофакторного анализа и обусловливается характером и сочетанием повреждений СКА КС, возрастом пострадавшего, его требованиями к функционированию поврежденного сустава и степенью готовности пациента к проведению реабилитационного периода и т.п.

4. Консервативное лечение показано при: изолированных полных повреждениях одной из крестообразных связок или аналогичных повреждениях, в сочетании с повреждениями СВО у лиц, не предъявляющих повышенных требований к функции нижней конечности; при частичных повреждениях одной из крестообразных связок верифицированных результатами МРТ или артроскопии; всех острых повреждениях СВО, за исключением повреждений с отрывными переломами со смещением; всех повреждениях СКА КС в сочетании с переломами проксимального эпифиза большеберцовой кости.

5. Оперативное лечение показано при: изолированных или сочетающихся с повреждениями СВО повреждениях ПКС у пациентов с высокой степенью физической активности; повреждениях крестообразной (крестообразных) связок у тех пациентов моложе 60 лет, у которых при МРТ или артроскопическом исследовании определяется возможность проведения органосохраняющей операции на крестообразной (крестообразных) связках КС; наличии изолированных или сочетающихся с повреждением одной (обеих) крестообразных связок повреждений СНО и/или стабилизаторов задне-наружного отдела КС; при отрывных переломах мест прикреплений боковых стабилизаторов КС; повреждении СВО у больных, ранее перенесших травмы КС, с отсутствием одной из крестообразных связок.

6. Относительными показаниями к оперативному лечению в остром периоде травмы является наличие: изолированных полных отрывов (разрывов) стабилизирующих элемен )в внутреннего отдела сустава у лиц молодого возраста, предъявляющих повышенные требования к функционированию коленного сустава; изолированные или сочетающиеся с повреждениями СВО повреждения ПКС у пациентов старше 50 лет с высокой степенью физической активности; повреждения СКА в сочетании с диафизарными переломами бедра и (или) голени на одной стороне.

7. Для объективизации выбора тактики лечения повреждений крестообразных связок коленного сустава в остром периоде травмы рекомендуется использование количественных показателей структуры тензоров напряжений и деформаций сухожильных и связочных элементов КС.

8. Выбор лечебной тактики при наличии клиники острых повреждений задне-боковых отделов сустава определяется характером внутрисуставных изменений, выявленных при комплексном обследовании пострадавших. При соче-танных разрывах (отрывах) крестообразных связок и стабилизаторов внутреннего отдела показано восстановление крестообразных связок (рефиксация, аутопластика, эндопротезирование), при обнаружении во время артроскопии разрыва капсулы и стабилизаторов внутреннего бокового отдела сустава рекомендуется выполнение чрезкожного сшивания зоны разрыва под артроскопическим контролем.

9. При острых повреждениях наружного и задне-наружного отделов коленного сустава целесообразно выполнение аутопластики свободными трансплантатами из сухожилий полусухожильной и тонкой мышц. При вывихах голени с повреждением наружного отдела СКА КС методом выбора должно быть раннее оперативное лечение с восстановлением задней крестообразной связки и элементов задне-наружного отдела сустава.

10. При отрывных переломах головки малоберцовой кости, внутреннего надмыщелка бедра или бугорка илиотибиального тракта большеберцовой кости со смещением показана ранняя рефиксация отломка; при сочетании этих травм с повреждением крестообразной (крестообразных) связок рефиксация костного фрагмента должна выполняться в качестве завершающего этапа оперативного вмешательства.

11. С целью объективизации оценки результатов реконструктивных и восстановительных операций при повреждениях крестообразных связок рекомендуется использование интегральных морфоденситометических показателей архитектоники элементов соединительной ткани.

12. Предложенная программа консервативного функционального лечения повреждений связочно-капсульного аппарата коленного сустава и лечебных мероприятий в пред-, послеоперационном и реабилитационном периодах, направленная на отказ от внешней иммобилизации и сокращение сроков лечения, рекомендуется к использованию в амбулаторных и стационарных медицинских учреждениях.
